Bumper to Bumper is seeking several skilled and energetic Delivery Drivers to join our team. Our Delivery Drivers are primarily focused on delivering auto parts to our customers.As part of the application process for employment at Moog Louisville WarehouseBumper to Bumper, I understand that they andor their agents may conduct an investigation of my personal information.

The investigation might include, but is not limited to, obtaining a copy of a consumer report prepared by a consumer reporting agency also known as a credit report and a backgroundcriminal records report, and investigation of previouscurrent employment, work experience, workers compensation claims, criminal history records state, Federal and other agencies, motor vehicle records, military records, education records, credit history and bankruptcy records.

I understand that information from these records and investigations may be used to determine my eligibility for employment, but will not be used in violation of any federal or state law or regulation. I hereby authorize without reservation the full release of these records. I also understand that the Company may require me to submit to a clinical test for the presence of alcohol andor drugs.

Before taking any adverse action based on the credit reports, including denying your employment, the company will provide to you, without charge, a copy of the credit reports plus a written summary of consumer rights under the Consumer Credit Reform Act.
